Glycemic Thresholds for Hair Transplantation: An Evidence-Informed Framework for Follicular Unit Excision

ABSTRACT

Introduction: Follicular unit excision (FUE) hair transplantation entails the creation of hundreds to several thousand puncture wounds in a vascularized scalp tissue bed, producing a wound environment physiologically susceptible to the impaired healing mechanisms of diabetes mellitus. Despite the clinical relevance of preoperative glycemic control to graft survival, no specialty-specific guidance exists on acceptable preoperative glycated hemoglobin (HbA1c) levels for FUE candidates with diabetes.

Methods: A narrative review of published guidelines, consensus statements, and peer-reviewed literature from 2018–2025 was conducted across perioperative medicine, anesthesiology, dermatologic surgery, wound care, and plastic surgery. Databases searched included PubMed/MEDLINE, the Cochrane Library, and Google Scholar using the search terms; the full search strategy is provided in the Methods section. Guideline bodies reviewed included the American Diabetes Association (ADA), the UK Centre for Perioperative Care (CPOC), the Society for Ambulatory Anesthesia (SAMBA), the American Society of Plastic Surgeons (ASPS), the Wound Healing Society (WHS), and the European Society of Cardiology (ESC).

Results: Across the specialties reviewed, preoperative HbA1c thresholds consistently cluster between 7.0%–8.5%, with both CPOC and ESC carrying recommendations for referral or deferral at or above 8.5% and the majority of plastic surgery and dermatologic surgery practitioners targeting values at or below 7.5%–8.0%. Meta-analytic data from elective abdominal surgery demonstrate a statistically significant increase in wound infection risk at HbA1c values exceeding 7%, with risk escalating continuously rather than at a single cutoff.

Conclusion: To date, the author is not aware of any studies that have directly evaluated HbA1c as a predictor of graft survival in FUE. A tiered, risk-stratified HbA1c framework is offered for community consideration in FUE patient selection: proceed with standard precautions below 7.0%; proceed with optimization and multidisciplinary coordination between 7.0%–7.9%; exercise caution with documented shared decision-making between 8.0%–8.4%; defer for glycemic optimization between 8.5%–9.4%; treat values at or above 9.5% as a relative contraindication. Consideration for adoption of this framework by hair transplant surgeons supports consistent patient selection, informed consent, and patient safety.
